Do you want to be part of a new service that will improve mental health outcomes for children and young people with learning disabilities and/or autism? Do you enjoy working with children, young people and their parent carers? Do you want to support children, young people and their families to create a personalised plan that ensures services work in a joined-up way that works for them? If so, this may be the opportunity for you! 

The Keyworker team will work to ensure children and young people with the most complex needs, initially those at risk of admission or in hospital, receive appropriate personalised, integrated support. This might be supporting someone at a crisis point after they have attempted suicide or serious self-harm injuries. You will do this by:

• Building a trusting relationship with the young person and their family through regular and consistent communication,
• Understand the services involved in their care and where there may be gaps,
• Provide emotional and practical support with the aim of improving the outcomes for the individual,
• Attend meetings (CETRs, LAEPs and other relevant multi-disciplinary meetings) alongside or as a representative for the young person to champion their voice and their wishes,
• Working with the young person and family to create a personalised plan that takes their goals into account alongside a multiagency approach including: Health, Social Care and Education,
• To “step away” once the positive outcome has been achieved and the young person/family are confident to continue without support.
